OCR description
The covered entity (CE), Huntridge Family Clinic Foundation, reported that a member of its board removed a box containing the protected health information (PHI) of 2,000 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license information, Social Security numbers, claims information, financial information, diagnoses/conditions, and other treatment information. Following OCR’s investigation, it was determined that this incident did not constitute a breach of PHI.
