OCR description
The covered entity (CE), Franciscan Alliance, Inc., reported that an employee impermissibly accessed the protected health information (PHI) of 2,180 individuals. The PHI involved included names, addresses, birthdates, drivers’ license numbers, health insurance information, Social Security numbers, financial information, lab results, medications prescribed, diagnoses/conditions, and treatment information. The CE notified HHS, affected individuals, the media, and local law enforcement. In its mitigation efforts, the CE sanctioned the employee involved and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
