OCR description
Multnomah County Department of Human Services, the covered entity (CE), reported that an employee impermissibly accessed the electronic protected health information (ePHI) of 713 individuals. The ePHI involved included names, Social Security numbers, and identification numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE retrained its workforce members. As a result of OCR’s investigation, the CE sanctioned the responsible employee and implemented additional technical safeguards.
