Ameritas Life Insurance Corp.

The covered entity (CE), Ameritas Life Insurance Corporation, reported that multiple employees were subjected to an email phishing scheme that affected the protected health information (PHI) of 69,106 individuals. The PHI involved included names, addresses, e-mail addresses, dates of birth, Social Security numbers, member identification numbers, and life insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on email security precautions and on the proper methods of identifying fraudulent email communications.
