St. Croix Hospice

St. Croix Hospice, the covered entity (CE), reported that several employees were the victims of an email phishing attack that affected the protected health information (PHI) of 21,407 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license information, Social Security numbers, claims information, financial information, diagnoses/condition, lab results, and medications prescribed. The CE notified HHS, affected individuals, and the media. The CE also notified state regulators as well as the major consumer reporting agencies. As a result of the breach, the CE implemented additional technical safeguards to better protect its PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
