OCR description
Comprehensive Cancer Centers of Nevada (CCCN), the covered entity (CE), reported that an unknown individual burglarized its locked storage trailer that contained the protected health information (PHI) of 2,251 individuals. The PHI involved included names, dates of birth, and clinical information. CCCN notified HHS, affected individuals, the media, and provided substitute notice. CCSN also provided free credit monitoring services to affected individuals. In its mitigation efforts, CCSN implemented additional administrative, physical, and security safeguards to better protect its PHI. During its investigation, OCR provided CCCN with technical assistance on the Breach Notification Rule.
