City of Philadelphia

The City of Philadelphia, the covered entity (CE), reported that an unencrypted laptop of an employee was stolen after the employee inadvertently left it on a public bus. Access logs to web-based applications that stored protected health information (PHI) revealed no unauthorized access to its computer systems. The breach affected approximately 1,500 individuals. The protected health information potentially disclosed included names, dates of birth, and treatment information. Following the breach, the covered entity ensured that all previously unencrypted mobile devices were encrypted. Additionally, OCR reviewed the covered entity’s risk analysis to ensure compliance with the HIPAA Security Rule.
