Fordland Clinic Inc.

The covered entity (CE), Fordland Clinic, reported that an employee was the victim of a phishing attack that allowed an unauthorized individual access to an email account containing the protected health information (PHI) of 881 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license information, social security numbers, financial information, prescribed medications, diagnoses/conditions, and lab results. The CE notified HHS, affected individuals, and the media. Following the breach, the CE immediately implemented additional technical and administrative safeguards to further protect its PHI. All employees received additional training regarding the risks associated with phishing and social engineering and ways to detect these schemes. OCR obtained assurances that the CE implemented the corrective actions noted above.
