The covered entity (CE), University of Missouri Health Care, reported that several employees were the victims of an email phishing attack that affected the electronic protected health information (ePHI) of 14,402 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses and conditions, lab results, medications prescribed, financial and health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also established a dedicated call center for questions or concerns. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on email security.
