OCR description
The covered entity (CE), Quorum Health, reported that a former employee sent emails containing the electronic protected health information (ePHI) of 1,269 individuals to her personal email account as well as a family member’s email account. The ePHI involved included names, birthdates, addresses, diagnoses, and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and security safeguards and retrained its staff. OCR obtained assurances that CE implemented the corrective actions noted.
