OCR description
The covered entity (CE), Bayview Dental, reported that it experienced a breach that affected the electronic protected health information (ePHI) of 1,938 individuals. The ePHI involved included names, dates of birth, Social Security numbers, addresses, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its administrative, technical, and security safeguards to better protect its ePHI.
