OCR description
The covered entity (CE), NorthStar Anesthesia, P.A., reported that a subcontractor of its business associate (BA) mailed documents containing protected health information (PHI) to the wrong recipients. This breach affected 19,807 individuals and the PHI involved included names only. The CE notified HHS, affected individuals, and the media.
