OCR description
The covered entity (CE), Premier Family Medical, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 320,000 individuals. The PHI involved included names, addresses, drivers’ license and Social Security numbers, diagnoses, claims and financial information, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protects its PHI. OCR provided technical assistance regarding the HIPAA Security Rule.
