The covered entity (CE), Berry Family Services, Inc., reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 1,751 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, and clinical information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring and identity protection services to affected individuals. In response to the breach, the CE created new policies and procedures to prevent future incidents of this nature and implemented additional administrative and technical safeguards. OCR obtained assurances that the CE implemented the corrective actions noted.
