OCR description
The covered entity (CE), June E. Nylen Cancer Center, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 927 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, and other treatment information. The CE notified, HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE hired a third-party to investigate the breach and provided additional HIPAA training to its employees.
