OCR description
The covered entity (CE), National Imaging Associates, reported that a former employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 589 individuals. The ePHI involved included names, dates of birth, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. Following the breach, the CE strengthened its technical safeguards and retrained its staff on email security.
