OCR description
The covered entity (CE), Wood Ranch Medical, experienced a ransomware attack on its computer systems (its network server and backup hard drives) that blocked access to the electronic protected health information (ePHI) of 5,835 individuals. The ePHI included demographic, clinical, and medical insurance information. The CE provided breach notification to affected individuals, HHS, and the media. Following the breach, the CE was unable to recover and restore the servers containing the PHI. As of December 17, 2019, the CE is closing its practice and ceasing operations.
