OCR description
Perfect Teeth Yale, PC, the covered entity (CE), reported that a storage device used to backup patient X-rays was missing. This breach compromised the electronic protected health information (ePHI) of 15,854 individuals. The ePHI involved included names, dates of birth, gender, and other identifiers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its employees to better protect the privacy of its ePHI. During the investigation, OCR provided technical assistance regarding requirements of the Breach Notification and Security Rules.
