OCR description
The covered entity (CE), Arkansas Health Group, reported that its business associate (BA), Arkansas Answering Service, sent protected health information (PHI) via email that was unsecured. The CE notified HHS, affected individuals, and the media. This breach affected 941 individuals and the PHI involved included names, birthdates, and clinical information. As a result of this breach, the CE terminated its relationship with the BA.
