Abdul Memon, MD, PLLC, the covered entity (CE), reported that an intruder burglarized his office and stole four laptops which contained the electronic protected health information (ePHI) of approximately 1,000 individuals. The ePHI involved included names, Social Security numbers, addresses, dates of birth, diagnoses/conditions, lab results, and medications prescribed. The CE notified HHS, affected individuals, and the media. Following OCR’s investigation, the CE revised its HIPAA policies and procedures, retrained its workforce members, and implemented new physical safeguards. OCR obtained assurances that the CE implemented the corrective actions noted.
