OCR description
Central Valley Regional Center, the covered entity (CE), reported that it was the victim of a cyber-attack affecting the electronic protected health information (ePHI) of approximately 15,975 individuals. The ePHI involved included names, addresses, birthdates, dates of death, Social Security numbers, drivers’ license information, and health insurance information. The CE notified HHS, affected individuals, and the media. Following OCR’s investigation, the CE implemented new technical safeguards and retrained its workforce members.
