LaBorde Therapy Center LLC

The covered entity (CE), LaBorde Therapy Center, LLC, reported that it an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 7,000 individuals. The ePHI involved included names and e-mail addresses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on recognizing and responding to fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
