OCR description
The covered entity (CE), Buckhead Smile Center, reported that an employee improperly emailed the protected health information (PHI) of 1,655 individuals to his personal email account. The PHI involved included names, diagnoses, Social Security numbers, dates of birth, addresses, and financial information. The CE notified HHS, affected individuals, the media, local and federal law enforcement entities, and provided substitute notice. A call center was established for questions or concerns. In its mitigation efforts, the CE implemented additional administrative safeguards to better protect its PHI.
