OCR description
The business associate (BA), Chronic Care Management (CCM), reported that it emailed the protected health information (PHI) of 612 individuals to the wrong recipients. The PHI involved included names, email addresses, and clinical information. CCM notified HHS, affected individuals, the media, and posted a substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative safeguards and retrained its staff. OCR obtained assurances that the BA implemented the corrective measures noted.
