Kalispell Regional Healthcare

Logan Health, formerly known as Kalispell Regional Healthcare, the covered entity (CE), reported that employees were the victims of an email phishing attack that compromised the protected health information (PHI) of 149,661 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, email addresses, claims information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical and administrative safeguards. OCR also provided the CE with technical assistance regarding its HIPAA Security Rule and Breach Notification Rule obligations.
