OCR description
Utah Valley Eye Center, the covered entity (CE), reported that it was the victim of a cyber-attack involving the electronic protected health information (ePHI) of 20,418 individuals. The ePHI involved included names, email addresses, addresses, and phone numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff. OCR provided technical assistance to the CE regarding the HIPAA Rules.
