Washington University School of Medicine, the covered entity (CE), reported that an employee’s personal laptop was compromised and used to access the employee’s email account which contained the protected health information (PHI) of 3,237 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. Staff were retrained on the requirement to protect and secure PHI.
