The covered entity (CE), Human Services, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 1,559 individuals. The PHI involved included names, addresses, dates of birth, and drivers’ license and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided complimentary identity theft protection services. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. The staff were also retrained on email security.
