Ivy Rehab Network, Inc. and its affiliated companies

The covered entity (CE), Ivy Rehab Network, Inc., reported that multiple employees were the victims of an email phishing attack that affected the electronic protected health information (ePHI) of 125,000 individuals. The ePHI involved included names, addresses, dates of birth, financial information, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthen its administrative and technical safeguards and retrained all workforce members on the proper methods of recognizing and responding to fraudulent email communications.
