Equinox, Inc.

The covered entity (CE), Equinox, Inc., reported that several employees were the victims of an email phishing scheme that compromised the protected health information (PHI) of 1,021 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, claims information, medical record numbers, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. Moreover, the covered entity retrained its staff on email security.
