OCR description
The covered entity (CE), Family Care Specialists Medical Group, reported that employees mailed protected health information (PHI) to the wrong addresses. This breach affected 2,490 individuals and the PHI involved included names and addresses. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative safeguards to better protect its PHI.
