OCR description
Starmount Life Insurance Company, the covered entity (CE), reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 563 individuals. The ePHI involved included names, dates of birth, Social Security numbers, financial information, and member identification numbers. The CE notified HHS, affected individuals, and the media. As a result of the breach, the CE implemented additional administrative and technical safeguards to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
