OCR description
Aetna – Affiliated Covered Entity, the covered entity (CE), reported that its business associate (BA), National Imaging Associates, was the victim of an email phishing scheme affecting the protected health information (PHI) of 5,991 individuals. The PHI involved included names, email addresses, member identification numbers, health insurance information, and claims information. Aetna notified OCR, affected individuals, and the media.
