Therapeutic Oasis of the Palm Beaches LLC

The covered entity (CE), Therapeutic Oasis of the Palm Beaches, LLC, reported that its office was burglarized and three computers were stolen, one of which contained the electronic protected health information (ePHI) of 1,100 individuals. The ePHI involved included names, birthdates, email addresses, and clinical information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Local law enforcement was also notified. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. All staff were retrained on the HIPAA Rules. OCR obtained assurances that the CE implemented the corrective actions noted.
