OCR description
The covered entity (CE), North Ottawa Community Health System, reported that an employee impermissibly accessed the electronic protected health information (ePHI) of 4,013 individuals. The ePHI involved included names, dates of birth, Social Security numbers, drivers’ license numbers, diagnoses and conditions, lab results, and medications prescribed. The CE notified HHS, affected individuals, and the media. Following the breach, the CE sanctioned the responsible employee, implemented additional administrative, technical, and security safeguards, and retrained its workforce members on safeguarding ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
