OCR description
The covered entity (CE), Texas Children’s Hospital, reported that a printing error caused protected health information (PHI) to be mailed to the wrong recipients. This breach affected the PHI of 597 individuals. The PHI involved included names and addresses. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative safeguards and retrained its staff.
