OCR description
Zuckerberg San Francisco General Hospital, the covered entity (CE), reported that an employee improperly disposed of documents containing the protected health information (PHI) of 1,174 individuals. The PHI involved included names, birthdates, clinical information, and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative safeguards and retrained its staff.
