OCR description
The covered entity (CE), Texas Family Psychology Associates dba Compass Psychological Associates, reported that a spouse of a former employee accessed the protected health information (PHI) of 12,000 individuals. The PHI involved included names, dates of birth, addresses, and medical information. The CE notified HHS, local law enforcement, and the FBI. In response to the breach, the CE implemented additional administrative and technical safeguards, and retrained its workforce members regarding security awareness. OCR provided technical assistance to the CE regarding the HIPAA Rules.
