OCR description
The covered entity (CE), Roosevelt General Hospital, reported that it was the victim of a cyber-attack that affected the protected health information (PHI) of 28,847 individuals. The PHI involved included names, addresses, dates of birth, and radiology treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
