Phoenix Children’s Hospital

Phoenix Children’s Hospital, the covered entity (CE), reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 1,860 individuals. The ePHI involved included names, dates of birth, addresses, drivers’ license information, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. As a result of OCR’s investigation, the CE revised its HIPAA security procedures, retrained its workforce members, and implemented additional technical safeguards to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
