UnitedHealth Group Health Plan Single Affiliated Covered Entity

The covered entity (CE), UnitedHealth Group Health Plan Single Affiliated Covered Entity, reported that an employee inadvertently sent a piece of mail that contained the protected health information (PHI) of 934 individuals to a plan member. The PHI involved included names and dates of birth. The CE notified HHS and the affected individuals of the breach. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative safeguards to better protect PHI.
