Village Center for Care d/b/a VillageCare Rehabilitative and Nursing Center

The covered entity (CE), Village Center for Care dba VillageCare Rehabilitative and Nursing Center, reported that an employee was the victim of an email phishing attack that affected the protected health information (PHI) of 674 individuals. The PHI involved included names, birthdates, and financial and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE sanctioned the responsible employee, implemented additional technical safeguards, updated its security policies and procedures, and retrained its workforce on email security.
