Personal Touch Hospice of VA, Inc.

The covered entity (CE), Personal Touch Hospice of VA, Inc., reported that its business associate (BA) was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 1,657 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE did not renew its contract with its BA. In its mitigation efforts, the CE implemented additional technical and security safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
