Lincoln National Life Insurance Company and its affiliate Lincoln Life & Annuity Company of New York

Lincoln National Life Insurance Company, the covered entity (CE), reported that an employee was the victim of an email phishing scheme affecting the protected health information (PHI) of 2,468 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, and insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented administrative and technical safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions discussed above.
