OCR description
The covered entity (CE), SAMA Healthcare Services, reported that its business associate (BA) spilled documents onto a road which contained the protected health information of 7,524 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses, laboratory results, and medication information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. Following the breach, the CE terminated its contract with the BA. OCR provided technical assistance to the CE and obtained assurances that the CE implemented the corrective actions noted.
