OCR description
Walmart, Inc., the covered entity (CE), reported that a set of pharmacy prescription records containing the protected health information (PHI) of 3,606 individuals, were missing. The PHI involved included names, addresses, phone numbers, dates of birth, and medication prescribed. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented new administrative safeguards and retrained it staff on the importance of safeguarding sensitive data.
