OCR description
The covered entity (CE), Tennessee Orthopaedic Alliance, reported that several employees were the victims of an email phishing scheme that compromised the protected health information (PHI) of 81,146 patients. The PHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
