Lake County Health Department

The covered entity (CE), Lake County Health Department, reported that an employee sent an unencrypted email to his personal email account that contained the electronic protected health information (ePHI) of 14,488 individuals. The ePHI involved included names and other identifying information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the responsible employee and implemented additional administrative, technical, and security safeguards to better protect its ePHI. All workforce members were retrained. OCR obtained assurances that the CE implemented the corrective actions noted.
