Munson Healthcare

The covered entity (CE), Munson Healthcare, reported that several employees were the subjects of an email phishing incident that affected the protected health information (PHI) of 75,202 individuals. The PHI involved included names, dates of birth, health insurance information, medical record numbers, prescription information, diagnoses, Social Security numbers, financial information, drivers’ license numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and established a call center for questions or concerns. In addition, the CE implemented additional administrative and technical safeguards to better protect its PHI. Staff were retrained on email security.
