OCR description
Alameda Alliance for Health, the covered entity (CE), reported that its business associate (BA) was the victim of a cyber-attack affecting the electronic protected health information (ePHI) of 500 individuals. After further investigation, the CE determined that none of its ePHI was affected by this breach incident. This case was therefore closed.
